A recent study found that among patients discharged from a skilled nursing facility to home, greater than 25% had negative outcomes in 30 days and over half within 90 days. What can be done about these unfortunate statistics? Connect-Home! Connect-Home, an LSA partnership with the University of North Carolina at Chapel Hill, is designed to strengthen the delivery of transitional care in acute care and rehab settings. It has been shown to improve patient and caregiver experiences of care and preparedness for discharge. Additional work in progress indicates Connect-Home is sustainable and contributes to continued care at home without hospital readmission.
